Parent Information Sheet This form is to be completed prior to the initial interview in the CSE Office. The information requested is used to set up the case in the office. Complete and accurate information on this form will allow us to work the case more efficiently. .DOC .PDF

Financial Affidavit
(If divorced or a divorce action
is pending with the courts)

The courts consider the income and certain expenses of the parties when determining the proper amount of child support to award in a court order. A financial affidavit provides the needed information to accomplish that. Income will be verified so it is important to accurately complete this document. If the parents are divorced or have filed for a divorce (actually completed paperwork and filed it at the courthouse), this form should be used. Otherwise, please refer to the other financial affidavit.
Note: All amounts listed should be converted to monthly figures. Please refer to the conversion chart.

The courts consider the income and certain expenses of the parties when determining the proper amount of child support to award in a court order. A financial affidavit provides the needed information to accomplish that. Income will be verified so it is important to accurately complete this document. If the parents were never married, or are married and separated but not divorced and no divorce papers have been filed with the court, or the caretaker/guardian of the child(ren) is not a parent, this affidavit should be completed. For caretaker relatives, the affidavit should be completed for the child(ren)'s income and expenses only since the caretaker relative or guardian's income and expenses are not considered.

Registration Statement This form is required in order for us to request that another state enforce our local order. When the payor leaves the state of Florida, we can request that another Child Support Enforcement Office enforce the order in their state. We must send this form along with other documents in order to request their services. .DOC .PDF
Change of Address OR Employment This form may be submitted by either party on a Child Support Case. This information is needed to be sure that we have the correct address for mailing correspondence and Child Support payments. Most court orders require the parties to notify the court of changes of address and employment within 7 days of the change. If there is a court order for support, the change of address will be provided to the depository and the enforcement department as appropriate. Note: Certain enforcement activities may still occur even without a current address, and failure to provide a current address may result in no notification to the party adversely affected. .DOC .PDF
